To Make an Appointment

Intake Form
All prospective patients must first fill out and submit the proper “Intake Form. Please fill out and submit for us to contact you to make an appointment.

If you do not have computer access or would prefer, you can call (334) 262-7787 and select the new client/general information extension. The intake coordinator will assist you by taking your information (by phone, this takes approximately 15 minutes).

If you would like to see a specific counselor, be sure to list their name on your intake form.

If you submitted an intake form on this site, a coordinator will contact you to schedule an appointment, usually within 24 hours.

Following your intake, you will be assigned a counselor by the Clinical Director, taking into consideration your needs and health insurance (if any).

You will receive an email from the center’s intake coordinator after you have scheduled your first appointment for counseling services. The email will have the date and time of your appointment as well as important information on completing required paperwork for your visit.

 

Forms for Your First Visit

All new clients should read the “Informed Consent” and the “Privacy Practices” prior to their first visit. You can also find these in your portal.

You will be assigned through your portal different questionnaires you MUST complete before your first appointment. If this is not possible please let the front office know to get other accommodations.

PLEASE BRING A LIST OF CURRENT MEDICATIONS (dosage and prescriber)

In addition, the client should bring a photo ID and Insurance Card or military ID (when applicable).
